Across the top is where the title of your survey would usually reside. Also your logo if desired.

Usually this is a place where we welcome the respondent, issue instructions or provide links for paper versions of the survey.

There are a number of logon options available:

• No logon required
            - this allows anyone to access and respond to your survey
• One username and password
            - this allows you to control who responds while retaining complete anonymity
• Username and password per department
            - this option provides a degree of accountability
            - problems/issues can be tracked to a group
• Username and password per respondent
            - this option prevents multiple responses from one person
            - easier to help with specific problems
            - survey doesn't need to be completed in one sitting
